1Provided you're performing the normal activities of a person in good health and of like age on the date insurance would take effect and the contribution is paid within 30 days of being billed. When coverage becomes effective, you will receive a bill and your Certificate of Insurance with full terms and conditions of your coverage. You will have a 30-day "free look" to review and determine if it is right for you or to cancel the coverage without for a full refund of any premium paid.
2You must be insured for a minimum of $1,000 Long Term Disability in order to qualify for the remaining benefits of the Basic Protection Package.
3Future Purchase Option can be converted to LTD monthly benefit following 12 months of continuous coverage and provided your income can support the increased coverage.
4Disabilities resulting from routine pregnancy, delivery, and related medical condition are not covered until you have been insured under STDi Plan 1 or Plan 2 for 12 continuous months.
